The pair saw off Nicolas Almagro and Santiago Giraldo in their respective semi-finals in Vina del Mar, Mayer reaching the first ATP final of his career.
He was outclassed once he got there, though, Fognini asserting his superiority with two breaks in a first set in which he never faced a break point on his own serve.
The Italian made a superb finish to last year's claycourt season, winning his two previous tour titles in Stuttgart and Hamburg and reaching the final in Umag, meaning he was playing his fourth successive final on the surface.
He took only one of his eight break points in the second set but it was enough to close out victory.
Fognini said: "I couldn't ask for a better start of the season. It's the third title, it's important, and now I go to Buenos Aires with a lot of confidence."
