The world number six has withdrawn from the WTA Championships in Istanbul with a viral illness, raising question marks about her participation in the final, which will take place in Prague on November 3-4.
However, when the teams were announced on Wednesday afternoon, the former Wimbledon champion's name was part of the Czech line-up.
Kvitova said she would be visiting her doctor in the Czech Republic and would be keeping her "fingers crossed" about her final fitness.
Captain Petr Pala will be able to call up a replacement if Kvitova has to pull out.
Lucie Safarova, Lucie Hradecka and Andrea Hlavackova make up the home team. It is the same quartet which won April's semi-final against Italy.
Serbia are also unchanged. They will again be led by their former world number ones Ana Ivanovic and Jelena Jankovic.
Bojana Jovanovski and Aleksandra Krunic provided back-up for the tie, which will take place on an indoor hardcourt.
The Czechs are the defending champions but were Kvitova to withdraw, Serbia would likely become the favourites to win the trophy for the first time in their history.
