Tennessee led 20-7 at half-time thanks to touchdowns from Jared Cook, on an 18-yard pass from Jake Locker, and Will Witherspoon, who took an interception 40 yards to the end zone.
Rob Bironas also kicked two field goals as the Titans scented a shock result at the home of their in-form rivals.
But the Colts scored two touchdowns in the third quarter, Delone Carter going in on the ground from a yard out and Cassius Vaughan intercepting Locker and returning it three yards for the score.
Bironas' third field goal edged the Titans back in front early in the final quarter but Colts quarterback Andrew Luck - who had been picked off twice during a mixed display - led two scoring drives in the final seven minutes that both ended in Adam Vinatieri field goals to give his side their third victory in a row.
Indianapolis improve to 9-4 and in good shape for a wildcard into the play-offs, while Tennessee's season is over at 4-9.
