St Louis added the two-point conversion on a Sam Bradford pass to Lance Kendricks to level things at 10-10 before San Francisco kicker David Akers landed a 33-yard field goal with one minute and 34 seconds remaining to edge the visitors back in front at the Edward Jones Dome.
Akers missed wide right - only just - from 51 yards with four minutes and 11 seconds remaining in overtime.
