Brentford beat Swindon on penalties after thrilling 3-3 draw at Griffin Park to reach the League One play-off final.
With the aggregate score tied at 4-4 after a pulsating 120 minutes in the semi-final second leg, Simon Moore saved Miles Storey's penalty before Adam Forshaw fired in the winning spot-kick to seal the Bees' place in the Wembley final.
Adam Rooney's unfortunate own goal opened the scoring, before Clayton Donaldson's neat turn and shot doubled the Bees' lead. Rooney made amends moments later, putting the Robins back in it from close range.
Donaldson extended the home side's lead, finishing well after intercepting Gary Roberts' wayward pass, but goals from Joe Devera and a late Aden Flint header sent the tie into extra-time.
Yeovil struck a dramatic late winner to move a step closer to their first ever promotion to the second tier of English football.
Ed Upson nodded home with just six minutes remaining to complete the turnaround and sentence Sheffield United to a third successive season in npower League One.
Gary Johnson's men took just five minutes to wipe out the Blades' 1-0 lead when Upson's top-draw through ball teed up Kevin Dawson.
But after Jamie Murphy's curling effort crashed off the woodwork the Glovers' strike pair of Paddy Madden and James Hayter both missed fantastic chances to put the tie to bed.
Upson's 12th goal for the club teed up a pitch invasion and a clash with Swindon or Brentford at Wembley for a place in the npower Championship.
