New Forest boss Sean O'Driscoll earlier on Wednesday verified reports of his interest in the 31-year-old Wales defender.
And Collins has now flown back to the United Kingdom from Stoke's pre-season tour in the USA in order to sign a deal with Forest.
Nottingham Forest chairman Omar Al Hasawi said: "We are delighted to bring a player of Danny's experience and calibre to the club.
"We are sure he will prove to be an excellent addition to the squad Sean O'Driscoll is assembling for the challenges that lie ahead."
Collins signed for Stoke from Sunderland in August 2009 and went on to make 60 appearances during his three-year spell at the Britannia Stadium.
But he did not feature in boss Tony Pulis' plans last season and he was loaned to Ipswich Town before another temporary spell saw him help West Ham United to promotion.
Stoke said in a statement on their official website: "The club would like to wish Danny all the best for the future."
