Onions, who returned from a career-threatening back injury four years ago to resume his Test career with England and as a linchpin for Durham's County Championship-winning team, felt discomfort during the ongoing Division One match against Sussex at Hove.
Durham coach Jon Lewis reported the 31-year-old seamer is awaiting the results of scans, having managed to bowl only eight overs in Sussex's first innings.
"Graham has had a scan on his back at a hospital close to the ground," Lewis told the club's official website.
"It's been looked at by (physiotherapist) Nigel Kent, but we are still waiting for the radiographer to tell us the results.
"We're hoping it won't be a long absence, but he may require some time away from the field."
