Having lost the toss and been put in to bat, Nepal made 137-7 in their 20 overs, with Gyanendra Malla top-scoring with 47.
He put on 55 with captain Paras Khadka (31), while Andy McBrine, Kevin O'Brien and George Dockrell each took two wickets.
In reply, skipper William Porterfield (38) and Andrew Poynter (28) had Ireland going well at 78-2, before a slump to 102-5.
However, a sixth wicket partnership of 39 in just 2.5 overs between Gary Wilson and Stuart Thompson saw their side home to 141-5 and a five-wicket victory.
Thompson hit 21 not out from nine balls, including the winning four from the first ball of the last over, which followed back-to-back sixes from Wilson (29no) at the end of the 19th.
